Through relationship-based care, practical assistance, and culturally responsive guidance, our services are designed to reduce barriers, strengthen stability, and help community members navigate systems with dignity and confidence.
Key Services
Care Management
Our care management services provide client-centered support for individuals and families. We offer walk-in services to reduce barriers to access and respond to urgent concerns in real time. Care coordinators assist with interpretation, transportation, accompaniment to appointments, application support, and referrals, ensuring clients can effectively engage with health and social service systems.
Systems Navigation and Community Engagement
We support individuals and families in navigating systems that are often difficult to access or understand. Care coordinators help clients engage with healthcare providers, connect to legal services, apply for public benefits, secure housing, and navigate immigration systems. In addition to one-on-one navigation, our team conducts community outreach through tabling at community events and partnerships with local organizations. These outreach efforts increase awareness of available resources, build trust, and ensure services reach community members who may otherwise face barriers to care.
Lay Counseling
Care coordinators provide lay counseling to offer emotional support, guidance, and problem-solving assistance during challenging life circumstances. Coordinators receive formal certification and participate in ongoing training and supervision with licensed clinicians, ensuring services are delivered with professionalism, cultural responsiveness, and ethical care. This training equips coordinators with skills in crisis intervention and conflict resolution, allowing them to de-escalate stressful situations, support individuals and families during crises, and connect clients to appropriate follow-up care.
