CERI’s Care Coordination and Social Services provide holistic, accessible support to individuals and families facing multilayered challenges. Through relationship-based care, practical assistance, and culturally responsive guidance, our services are designed to reduce barriers, strengthen stability, and help community members navigate systems with dignity and confidence.

Key Services


Care Management & Systems Navigation

Client-centered support for individuals and families, we offer walk-in services to reduce barriers to access and respond to urgent concerns in real time. We help people access mental health and health care, housing, legal referrals, employment, and more. Care coordinators provide interpretation in 20+ languages and dialects, and also assist with transportation, accompaniment to appointments, application support, and referrals. We walk alongside individuals and families to navigate systems that are often difficult to access or understand, and meet with clients for as many appointments as needed until the challenge is resolved.


Community Engagement

Our team conducts community outreach through hosting or tabling at community events. These outreach and engagement efforts increase awareness of available resources, build trust, and ensure services reach community members who may otherwise face barriers to care.


Lay Counseling

Care coordinators are trained in lay counseling to offer emotional support, guidance, and problem-solving assistance during challenging life circumstances. Through formal certification as well as ongoing training and supervision, care coordinators are equipped with skills in crisis intervention and conflict resolution, allowing them to de-escalate stressful situations, support individuals and families with urgent needs, and connect clients to appropriate follow-up care.
